The Privacy Policy applies to the restorefunction.com (“restorefunction site”, “the site” or collectively “the Website”), including all of the information, services, and/or products (Collectively, the “Content”), provided by Restore Function Physical Therapy (“Restore Function, the “Company”, “We”, “Our”, and “Us”). The Privacy Policy is only applicable to the Website and not any other website (Third Party Website) that is accessible through this Website, that may differ from this Privacy Policy. You are subject to the Privacy Policy, Terms, and other Policies of the third party website when you leave the Website!

We appreciate the trust the User (“You”, Your”, the “Patient” and the “Client”) place in Restore Function and will respect and protect the privacy and security of your personal information when using Our Site. Our Privacy Policy describes the information, data collection, data storage and practices and policies regarding the use of information/data that was collected. Access and continued use of the Site confirms you have read, understood, and agree to be bound by the practices and policies of the Privacy Policy. Information Collected and Use Practice

Restore Function may collect Personally Identifiable Information (“PII”) and Non-Personally Identifiable Information (“Non-PII”) anytime you access the Site, communicate via In-person or telephonic conversations, online, and/or any other verbal or written communication.

PII Collected. Personally Identifiable Information that may be collected, without limitation, may include the following categories:

  1. Contact Data (Name, mailing address, telephone number, email address, personal data, corporate information)

  2. Financial Data (Credit card number and credit card account information)

  3. Demographic Data (Age, gender, marital status, zip code)

PII is collected at various times including, without limitation, when accessing and using the Site, participation in Services, downloading Content, contacting the Company or requesting customer support.

Non-PII Collected. Non-Personally Identifiable Information that may be collected, without limitation, may include: the device ID, IP address, browser types, plug-in details, and cookies. The Non-PII data may be used to identify performance concerns and problems in various sections or throughout the Site. Restore Function does not practice selling, renting, or trading personal information with other Third-Parties. We may disclose PII or other data if required to comply with law or other legal requirements, to assist in fraud protection or investigation, to enforce or apply the Terms and Agreements, and/or to protect the rights, property or safety of other Users. Data and information is not provided to third party companies for advertisement or marketing purposes.

Restore Function reserves the right to use and disclose Non-PII to third parties since it has no personally identifiable information to the User.

Email Marketing

If you submit your email address to Us, you consent to receive emails Restore Function concerning promotional offers and advertisements. Emails may be sent following a processed order, to assist and/or provide You with support. Emails sent to the User is only intended for the Users who request to receive emails. We provide instructions to opt-out and unsubscribe from the email marketing within our emails. Please allow ten business days in order to process your request to opt-out of future marketing emails.

Confidentiality and Security

Restore Function is committed to the security of Your PII from loss, misuse, alteration, unauthorized access, disclosure and/or destruction by using reasonable safeguards for all information collected. We use technology and processes to protect collected information, however no system is 100% safe and secure. We will continue to use and review all measures and safeguards and update them appropriately in order to protect Your information. By access, use of and transmission of any information through the Site, You do so at your own risk.

Privacy of Children. The Content on the site is intended for users at least 13 years or older; however children of all ages may use the Content of the Company and site with expressed permission by parent or legal guardian. The Company does not knowingly collect personal information from any persons under the age of 18 and/or legal age of adulthood in specified Country. Additionally, if You are a child under the age of 18 please do NOT submit any personally identifiable information, otherwise the Site will delete all information from Our records.

Users Control Over PII. At any time, but only once per calendar year, or as otherwise required by applicable law, Users may contact Us at Wesleyscott@restorefunctionpt.com to review the PII the company has accessed from You, update/modify Your PII, and/or request your PII be removed by Restore Function. Please submit an email to the email previously stated with the subject line of the email as follows, “Personally Identifiable Information Review.”

California Privacy Rights. Any and all California Residents are permitted to request any disclosed personally identifiable information from Our Site to third parties for direct marketing services under California Civil Code Sec. 1798.83. Any change, modification, and/or update will take into effect immediately, upon their posting to the Website.
